Most of the time, it is a bad temperature sender. Most cars have two of them, one for the gauge or light and one for the fan. One is usually by the thermostat housing and the other should be on the head somewhere. The one for the gauge should have two wires to it and the one for the fan only one. You can unplug the wire running to the sender with the one wire and ground it out with a wire or just touch it to the engine. If the fan comes on, that's the problem. Cars vary, so the switch should be in the run position, but the car does not need to be running. Check the fuses first, both under the hood and inside. It can be a relay in the box under the hood also. You can check the fan by running power and ground to it to see if it runs.
