The method differs in different countries.
In many countries you prefix the country code by two zeros or a plus. then the country code then the telephone number, omitting any leading zeros in the area code.
So to call the Swedish meterological insitute in Norrköping Sweden from a United States land line you would dial:
00 (international)
46 (that's the country code for Sweden)
11 (the area code for Norrköping is 011- drop that initial zero)
495 80 00 (the number you would have dialed if you had been in the Norrköping area code)

No it is not a continuous variable because whilst you could have telephone numbers such as 123456 and 123457 you could not have a telephone number that was in between these two such as 123456.5.
