Near the end of the game, Ezio acquires a map revealing all of the codex pages. This is combined with your current map. Basically, the map you've always used will now show where the rest of the codex pages are located.

If all of them have been decrypted, as soon as you interact with the Codex Wall, they are placed in their proper order, only you need to rotate them to solve the world map (you need to have your Eagle Vision on to notice it and can only be done after all other missions are completed).
If you're not up to the last mission, then synchronize all the view points in every location. This may take a fairly large amount of time, but it will reveal the locations of all of the codex pages on the map (minus the ones you get in the story).
