First you have to place the chest somewhere where there won't be a block above it, then you right click on it. You can manually place things in there, or you can hold Shift and Left click an item to put it in the top-left most open space in the chest.

The Alchemy Chest is part of the equivalent exchange Mod, Technic and Tekkit so you can't make it in the vanilla minecraft.
