There are several things that have been implemented to limit this problem. First, there is "tapping." If you damage a monster first, you have thereby "tapped" it and thus only you and your party will gain experience and loot from it, even if another player attacks and kills it. If the nameplate on the monster is grey that means someone else has tapped it. Portions of the game are instanced, meaning only you and those in your group (or quest progression for some instanced zones) will be able to interact with you.

On PvP servers you can get camped, ganked, and do the same to others. GM's will seldom interfere in a situation that could be resolved through PvP unless it is bad enough to be considered griefing. On Normal servers this is not as big a problem as only those in certain zones or those flagged for PvP combat can participate in these activities. There are positive and negative things about both.

Spawn camping is very difficult since most monsters spawn in a variety of locations (with the exceptions of some quest NPC's and similar monsters) and players once killed can resurrect either at their recently killed corpse or the Graveyard. If you are killed too frequently you get a resurrection timer preventing you from immediately resurrecting after returning to your corpse. The maximum timer from this is six minutes - longer than the average attention span of even the most obnoxious campers.
