This is a little complicated due to the fact that they changed the rear wiper arm mid-model-year. If you have a rear wiper that looks all plastic and sort of finished and fancy, then you have the "early production" model. If the wiper is more metal and looks more like a conventional wiper blade, then you have the "late production" model. See sources and related links below to determine which kind you have. Lastly, if you have the early production type, save yourself some time and order it right away because they are nearly impossible to find in stores.
Since rear wiper blades undergo substantial abuse - especially in icy/snowy locations - and most vehicle owners do not replace the rear wiper very often, I highly recommend replacing the entire wiper blade assembly (blade and frame) vs. (rubber wiper refill blade). To replace the rear wiper blade on a Chrysler Pacifica, you must first remove the old blade. You can remove it by pushing the small blade that holds the wiper in place up, and push the blade back. Once the blade is free from the hook remove it. Put the new blade in position so the hook is in position to go over the wiper attachment. Clip it into position and you are done.

It is easiest to replace the back wiper blade by carefully prying off the cap at the bottom of the wiper and then remove the whole wiper arm. After that you will be able to easily replace the wiper blade per the instructions given by the wiper blade manufacturer. Once you have done that, simply reattach the arm and snap the cap back in place. This process will take you less than five minutes.
