First, you must obtain soul sand, a block found in the nether. Also, you must obtain wither skeleton skulls. You have to kill wither skeletons in the nether for them to drop wither skeleton skulls. However, it is a rare item and usually it doesn't drop.

To make the wither, place 2 soul sand blocks one on top of the other and 2 more soul sand on both sides of the top block, so it looks like a "T" at eye-level. Then, place 3 wither skeleton skulls on top of the soul sand blocks. The last block you place has to be a wither skeleton skull or else the wither will not spawn.
