Dry rice: will stay good for years if kept in the right conditions. Dry rice has gone bad if it begins to smell, feels moist when it should not, changes any color other than the original color, or contains a large quantity of obvious bugs (a small quantity could be removed safely)
Cooked rice: will stay good for a few days if properly contained and refrigerated. Cooked rice has gone bad if it begins to smell strange, changes colors, has obvious mold growth, or becomes slimy. If the cooked rice has merely become "dry" it is likely still safe to eat if rehydrated, though it may not recover its original texture (it tends to become mushy and tasteless when reheated from a dry state)
I know of two ways that brown rice can go bad:
If there are bugs or possible remnants of a bug presence, toss it.
You can smell it. If it smells rancid, you may want to replace it.
It will still be hard to bite into. - Undercooked.
It will be soft and mushy - Overcooked.

You can tell that a ham has gone bad by looking at it and smelling it. If it smells strange or looks like it has mold, it is bad.

I was looking for the answer to this question myself. I couldn't find a definitive answer however I went to monkeysee.com and Christine Bruhn (PhD & Director of the Center for Consumer Research at the University of California) says that if the fats in the brown rice is rancid, it becomes a carcinogen.
