You first make sure the socket in the head is clear and smooth. Fit your shaft into this socket and bang the other end on a firm surface, -the head will settle into position, but now needs to be pounded on with a soft face or dead blow hammer. When the end of the shaft is level with end of head you have it in position. Now you pound a small metal wedge into the end of the shaft to spread it. These wedges are available in all hardware stores in packs of 3-4 quite cheaply.

drill a hole in the center about 1/8 inch and with another hammer-- hammer a cement spike into the drill hole-- this will spread the wood center creating a compressed tight fit on the hammer head.

ANS 2 - Small metal wedges made for this purpose are sold cheaply in packages of 3 or 4 at most hardware stores. - No hole drilling, you just centre them and drive them in.

If the handle is broken then you would want to use wood glue to put the handle back on. If that doesn't work, then you could go back to your local craft store and get another handle. This would ensure that the handle is strong enough.
