An abscess is a sore on the gum that is caused by an infection. The only way to treat an abscess is to get an antibiotic from your dentist.
You would have a swollen lip, a possible abscess and nothing to show for it.

That shouldn't happen unless the piercing is currently infected. If for some reason you do get an abscess you'd just go and see a doctor.

I would do a water change. What it seems like to me is the fish has a blister or abscess forming on its mouth/lip. This is usually caused by poor water conditions.
Then if the lip was broken and swollen then you have give epinephrine shot.

It really just depends on how deep it is, and how you treat it.
The only way that I could see an abscess forming is if it gets infected and you don't treat it properly. If you take the jewelry out with an infection, you trap the infection and an abscess forms. Otherwise, you're good to go!
I do believe that you need to see a dentist and not an orthodontist regarding a tooth abscess. If anything, the dentist would treat it or if need be, refer you to a specialist.
