There are many different ways to write a diary entry. Typically, you would write it from the point of view of the character, about a problem in their life.
First, select a problem that you wish to talk about.
Introduction: Describe the problem - what is it?
Paragraph 1: How did it start? What happened in the beginning? Provide the background details
Paragraph 2: How do you feel about it? What are some conflicting ideas or feelings that the character is facing?
Conclusion: How are you going to solve it? What happens in the end? Do you know?
That really depends on what the entry is support to be about. If you're writing a review on the book then you should start by giving a brief summary of the plot, before giving your opinion on the plot, characters and general feeling about the book. You should then conclude it by summarising your main thought.
If you're trying to write an entry as though you were one of the characters, then you firstly need to decide which character you're going to be. Say you're writing from Ron Weasley's point of view. You need to make sure you get a good feel for him, the type of things he says, what he feels. Try imagine what he is thinking in a situation. Then decide which particular event you're going to have him write an entry about. Retell the story but from Ron's point of view. He likes to exaggerate a bit, so maybe Fluffy the three-headed dog was trying to big his ankles? Write about how he was feeling. If you were writing about Hermione, then perhaps she decides to tell her diary how stupid Harry and Ron were to sneak out of bed at night and put them in all in so much danger. The key is to try and understand the character whose point of view you are writing from.
Harry Potter didn't keep a diary. He only wrote in Tom Riddle's diary in an attempt to find out more about the Chamber of Secrets.
