A business letter of request should follow this general outline:

Address the person formally, such as: Dear Honorable Judge Markus B. Surname with a comma after the name. If you only have a title, use the title: Dear Recorder of Deeds with a comma after it.
Next, think about the purpose of the letter. This portion is unclear in your question here. Think about what you want to request, then, state it simply and clearly.
Consider whether you have other information to include, or specific limitations you have on your request, such as a desired deadline for a reply.
In business letters, always end by thanking the person/Official. Then, skip a couple lines, type the words Sincerely yours, and sign your name in cursive writing (American English, British English) or whatever is formal writing in your country.
Under your signature, print or type your name.
Note: Business or formal letters should be typed. If you have no way to type it, write very neatly.
