She shimmied up a tree to get some flowers and the branch broke and she fell in the river. She was too looney to realize that she was in danger of drowning so she eventually got pulled under by her heavy, wet clothes. At least, that's the way Gertrude tells it.

She was picking flowers and fell off a tree when the branch broke under her. Her dress acted as a flotation device at first, but after it had become soggy, it dragged her underwater. Ophelia apparently had very little idea of what was happening to her. It's really unfair that they did not give her a proper burial since she was clearly a looney and not a suicide.
