Goitre (a swelling of the thyroid gland) normally occurs when the body lacks iodine. The gland swells in order to try to extract more iodine from the bloodstream. This happens in upland areas well away from the sea. In the UK, it's known as Derbyshire neck. In mainland Europe it's known as Swiss neck. It is almost unknown now in Derbyshire or Switzerland except amongst people who live on farms and draw their water from private wells and iodine is added to the mains water supply. Goitre remains a problem in the Balkans, however, especially the Carpathian mountains, where the government has failed to provide iodine supplements or to add iodine to the water.
