I am not quite sure what is "usual" but what happens is that the president-elect comes up with a list of people he wants to fill the cabinet slots and after the people who are selected agree to serve, the list is sent to the Senate to be confirmed. The reasons behind the selections vary and are subject to speculation. Nowadays, ethnic diversity seems to be a requirement. Cabinet appointment may be used to repay political debts. Sometimes rivals are appointed to get them out of the way. Kennedy appointed his brother to head Justice.
Modern presidents often do not rely on their cabinet for advice. Most of the Cabinet members are chosen for political reasons, to reward people who were helpful in the campaign. They are also picked for ethnic diversity -- a black, a Jew, a woman, an Hispanic, etc. Some are picked for their administrative experience since they have to run their department. Some may be picked to satisfy a particular group within the party.
