This will vary from state to state, as each state's board of pharmacy may enact it's own rules. It may also depend on your insurance.

Generally, prescriptions are good for one year. Controlled substances must be filled within 30 days.

Note that your insurance plan may layer additional restrictions on top of those imposed by your state. For example, in New York state, medicaid requires a new prescription for a non-controlled substance to be filled within 60 days from the time it is written, or medicaid payment will be denied.

This depends on who you ask and policy of the institution you visit. I have heard many places that for controlled substances the prescription is valid for 6 months. However, for me personally from experience my pharmacy would not take a prescription I had that was over 60 days over for a narcotic. I had to have them fax the doctor's office for a new prescription. It doesn't hurt to just call the pharmacy.

It depends on what the doctor wrote on the prescription, a "valid until" date is almost always included. For controlled substances like strong pain medications it will only be good for a short time until the doctor suspects the pain will go away. Other prescription medications they may be good for up to a year, but the doctor will limit the number of pills you may get each month, you cannot get a years supply up front.
