In California, Blue Shield has a discount of 25% off of the usual and customary fee for acupuncture, but the patient pays out of pocket. Blue Cross depending on the plan, is usually covered through a subcontractor called American Specialty Health (ASH). Your acupuncture benefits would be through American Specialty Health, who would administer the benefits, basically they would tell you how many visits you would get. In my practice I am usually allowed to give 5-6 treatments and then the ASH would determine if the patient could get more acupuncture visits based on progress, the nature of the injury, and other factors. there is usually a cap on visits per year at 20 to 30 again depending on your plan specifics.
