All of the bodies were recovered, but it was never determined how the astronauts died. It is assumed they died due to the impact with the ocean, and not the explosion itself.
Nobody knows who died first in the accident. Even if it was, NASA would most likely not release the information out of respect for the astronauts and their families.
The Challenger was a space shuttle orbiter that took astronauts into space for studies. The Challenger completed a total of nine missions into space.
They didn't die during the blowup, they died when the part that they were in hit the water so hard that anybody could survive-
