In a higher-level youth orchestra or band, you'll get placed by how you perform in an audition, and the requirements for auditions vary insanely. In professional orchestras, you don't really get placed. A spot opens up--be it piccolo, first flute, second flute, assistant--and everyone who wants the job tries out, usually with several rounds of elimination, where the potential performers play concertos or solo pieces for judges. Even if one person comes out first out of all competitors, they may or may not get hired.
