Commonly a dose between 6 and 40 mg's a day is prescribed to treat chronic anxiety and convulsions in adults with tablets of 2-10 mg taken orally 3-4 times a day.
Theoretically there is a maximum dose which lies between 50-60 mg's, but this is NOT recommended and certainly not as a long term treatment.
For children older than 6 months Diazepam can be prescribed for the same symptoms varying in a dose from 6-10 mg a day. In case of anxiety always consider we're talking about children and medicines with such an impact should only be prescirbed when there's no other alternative. The same goes for the elderly people. In both these groups Diazepam might result in adverse reactions, so it should be built up and monitored carefully.
If 40 mg a day isn't enough to take care of your problems, consult your doctor or psychiatrist for an alternative like Xanax (Alprazolam) or Klonopin (Clonazepam/Rivotril).
Try to keep the treatment with all benzo's as short as possible. Tolerance develops itself soon, which will trigger anxiety between the doses. That might result in taking more of the medicine. The higher the dose, the worse the anxiety gets between the doses.
Antidepressants (SSRI's and TCA's) might help to deal with your anxiety too. Even though these medicines work for most people, i might not supress all anxiety. It might also give a agitated feeling as a side effect, which can be very annoying to people who suffer from anxiety dissorders. Another annoying side effect is the problem with sexual disfunction. You might experience less excitement, a delayed orgasm (which is positive for a girlfriend) or no orgasm at all. These effects are dose dependant and sometimes appear randomly. Important notice is that these medicines might worsen your problems the first 2-6 weeks. After that they will start to work. Most of the time the dose will be elevated after 8-12 weeks to reach an optimal result.
2 other medicines which are not approved for anxiety but are prescribed off label are Seroquel (XR) (risk of heart failure and Diabetes with long term use) and Pregabaline (Lyrica) This last works on GABA (just like benzo's like Valium) to block certain channels in the brain. It works for approximately 50% of the people with anxiety and there is no tolerance during long term use.
One last note: don't operate heavy machinery or drive a car in the first 2 weeks that you use the medicines above. If you NEED to drive a car after these 2 weeks try to see how it works out with someone next to you first. If you decide to drive alone after such a period always take a rest after a maximum of 2 hours. When you feel tired, look for a parking place to take a rest !
