I know it is outrageously expensive in the U.S. due to the 'free market' (approx. $100,000) but I have found that prices in India are much less.

Cost of Pacemaker implantation in India is very less in India as packages are for 9000-950o$. Packages includes catheterization room charge, accommodation charge , diagnostics charges, doctors fees, medical supplies etc.

The quality of the top hospitals which advertise for medical tourists is very good. Often the doctors have been trained in the U.S. or Europe.

We have not had 'free market' health care in America, so 'free market' is not why this cost is so high. There is a real 'free market' provider for this surgery in the US and their cost for a pacemaker replacement is $7,600. They are the Surgery Center of Oklahoma. Free market would mean that the entities driving the 'cost' is supply and demand with direct competition. What we have had is Medicare rates, insurance companies, and corporate hospital systems driving (and distorting) supply and demand while dampening the possibility of competition (let alone transparency).

I have one of the most advanced devices currently available so this woud be the most youd pay...

List price for a Secura DR with Sprint Quattro leads by Medtronic lists for about $40,000. Factor in operating costs and an overnight stay in the cardiac Ward and you're looking at about $90,000. However they do have a variety of assistance options to help.

What are Medicare approved fees for various services?

Medicare has limits on the amount of money they will pay for specific services. Only Medicare themselves have access to the actual dollar amounts.
