It condenses on the window. Evaporation is the change from a liquid to a gaseous state. For example water in a lake is warmed by the sun some of the water molecules receive enough energy to change into a gas (water vapor). When enough of this water vapor gets together it forms clouds.

Physical Change, the window is still a window and you can get it back to its original state by re-melting it (although i dont see why you would want to do this). No new substace was formed, nor anything given off. :)
Crystals are the form that minerals take upon solidification. Metamorphic is a term used to describe a rock that undergoes a chemical or mineral transformation from heat or pressure. New crystals can form in rock undergoing metamorphosis, but the crystals themselves are not considered metamorphic.
