The Bible's answer:
Matthew 18:15-17 - ""Moreover, if your brother commits a sin, go and reveal his fault between you and him alone. If he listens to you, you have gained your brother. But if he does not listen, take along with you one or two more, so that on the testimony of two or three witnesses every matter may be established. If he does not listen to them, speak to the congregation. If he does not listen even to the congregation, let him be to you just as a man of the nations and as a tax collector."
United States Law:
State laws vary as to whether you are required to report a crime. In the majority of states, failure to report isn't illegal, but some states have laws that punish individuals who fail to report certain types of crimes.
Generally, as to testifying in a criminal case, a court can serve you with a subpoena to appear as a witness to a crime. The subpoena must inform you as to what testimony is required. A witness can refuse to answer only under special circumstances.
Otherwise, you must testify. However, a witness with any issues should consult with an attorney before testifying.
