While in a vastly weakened state after having suffered a cut from a poisoned knife by the herald-traitor Nogusha, Druss suffers a lance blow from a Nadir warrior which crushes his ribs and punctures a lung, causing him to simultaneously bleed out and asphyxiate. Ulric mentions that he did not know Nogusha was poisoned, so Druss' death could be related to loss of blod, poisoning or simple inability to breathe due to the crushed long. Most probably, it is a combination of all three.
