Division of cytoplasm is called cytokinesis. It starts during the last stage of telophase. In plant cells, the cytoplasm divides by the formation of a cell plate, whereas the cytoplasm divides by the inward pinching or furrowing in an animal cell.

are you sure you didn't mean mitosis? if so it would be TELOPHASE where the chromotids reach the opposite poles, uncoil(now becoming chromosomes again)

the nuclear envelope forms around each group. then the cytoplasm divides giving two identically identical daughter cells
