Not very well I imagine, but well enough that they can get the point across. In any case, the average person in Japan will probably have a better grasping of English than the average Americans would have of Japanese.

My daughter lived in Japan for three years, teaching English as a second language, and I visited. I asked her this question while there, and she replied that about 40% of the people speak English. In fact, to practice, people on the street will often speak to foreigners just to practice their English. Japanese children begin learning English in elementary school, commonly in the third grade.
