Tinnitus is a symptom, not a disease. The perception of sounds like ringing, buzzing, or a cricket sound even in very quiet environments may be a symptom of a variety of disorders affecting the ears, the auditory nerve, or the brain. The first step for anyone with bothersome tinnitus is to seek out a professional, an audiologist or medical ear specialist (an otologist) who can perform a thorough hearing and tinnitus assessment. A small proportion of persons with bothersome tinnitus may be helped with medical treatment of the underlying disorder.

Most people with tinnitus do not have a disorder that needs medical management. Tinnitus generally occurs when there is dysfunction of tiny hair cells in the inner ear that may or may not be associated with a hearing loss. The two most common explanations for tinnitus are age-related changes in the ear or hair cell dysfunction following exposure to high intensity sounds.

Tinnitus is annoying for some persons. It may interfere with sleep and it reduces quality of life. Although tinnitus may be perceived for a long time, even years, it is not a permanent condition. With proper management, bothersome tinnitus may be minimized until it is barely noticed or no longer perceived at all. Persons with bothersome tinnitus should consult with an audiologist who has expertise in tinnitus assessment and management. All persons with tinnitus can be helped!

It depends on what the cause of the ringing in the ears is, there are many different causes. If it is due to neurological damage to the ear or hearing from such things as high noise levels (in single bursts or for sustained periods), injury, or due to the aging process, there is little that can be done. However, some causes are treatable and can eliminate the symptom of tinnitus.

Causes:

  1. Presbycusis also known as age-related hearing loss.
  2. Exposure to loud noise like from gunfire, manufacturing, heavy equipment and chain saws or like from overly loud decebels of music. Loud noises, such as those from heavy equipment, chain saws and firearms, are common sources of noise-related hearing loss.
  3. Earwax blockage can cause hearing loss or eardrum irritation, which can cause tinnitus.
  4. Otosclerosis, loss of flexibility of the small bones in your middle ear can be familial and is caused by abnormal bone growth.
  5. Meniere's disease (caused by abnormal inner ear fluid pressure).
  6. Stress and depression. For some reason, these conditions can be associated with tinnitus.
  7. TMJ disorders (problems with your jaw joint, the temperomandibular joint).
  8. Head injuries or neck injuries can cause disruption in the auditory nerves or the brain functions needed for hearing.
  9. Vestibular schwannoma also known as acoustic neuroma is a benign growth on the nerve that controls balance and hearing and can cause tinnitus (usually in one ear).
  10. Pulsatile tinnitus is caused by blood vessel disorders that can include:
    • Head and neck tumors that constrict or obstruct blood flow in your head or neck vessels.
    • Atherosclerosis causes major blood vessels to lose their elasticity and ability to expand slightly with each heartbeat which causes a need for more forceful flow and that allows you to detect your heart beats as ringing.
    • High blood pressure can make tinnitus more noticeable.
    • Turbulent blood flow caused by strictures of a carotid artery or the jugular vein can lead to tinnitus.
    • Arteriovenous malformations (AVM) can occur in the connections between arteries and veins causing tinnitus usually in only one ear.
  11. Medications can cause or worsen tinnitus, the higher the dose usually the tinnitus is worse. It can be relieved when the drug is stopped. Medications known to cause or worsen tinnitus include:
    • Antibiotics, especially chloramphenicol, erythromycin, gentamicin, vancomycin and bleomycin.
    • Cancer chemotherapy drugs, including mechlorethamine and vincristine.
    • Diuretics such as bumetanide, ethacrynic acid, furosemide.
    • Quinine (used mostly today for malaria).
    • Very high frequent doses of aspirin (12 or more a day).
    • Chloroquine (another medicine for malaria).

Treatments:

Some treatments for tinnitus that can cure or improve the problem are:

  1. Treating underlying health conditions that may be associated. Examples include:
    • Earwax removal.
    • Treating a blood vessel condition with medication, surgery, etc.
    • Stopping or reducing a medication or changing to a different drug.
    • White noise machines.
    • Hearing aids.
    • Masking devices worn like a hearing aid that generate continuous, low white noise.
  2. Noise suppression so you notice it less. Some ways are:
  3. Tinnitus retraining devices can be worn to produce tonal music that masks the specific frequency of the tinnitus that you have. Counseling is often a component of tinnitus retraining.

Medications may help but don't cure the problem; some used are:

  1. Tricyclic antidepressants, like amitriptyline and nortriptyline, may help but usually are only for severe cases since the drugs have untoward side effects in some people.
  2. Alprazolam (Niravam, Xanax) may help, but they, too have side effects like drowsiness and nausea and they can be habit-forming.
