No, cramps do not have to accompany a pregancy. Cramping in early pregnancy is not a good sign. It may indicate that a miscarriage is threatening and may be accompanied by bleeding.

Do not freak out about miscarriage unless the cramps are excruciating and accompanied by bright red heavy bleeding. Call your doctor just to make sure, if you are feeling like it is "bad".

Ectopic pregnancies are signified by severe pain on one side, usually the left, with shooting pains into the shoulder and chest. This is more likely with women with tubal blockages such as ligation or cysts, and family history. If you feel these things, get to the ER asap. Ectopic pregnancies occur elsewhere than in the uterus (in the tube, usually), and are life-threatening.

Spotting brown for a day (not enough to use more than a pantyliner (as well as pink mucous-like spotting) around the time of your period is normal, as it is likely implantation bleeding.

Cramping similar to milder than period-like cramps, is ABSOLUTELY normal. Some women experience this as early as the first week after ovulation, while others get it later. The cramps feel like period is coming, but are actually related to the uterus stretching and implantation. CRAMPS are NORMAL, unless it is excruciating.

If it is light cramping and not accompanied by bleeding, they are normal and are caused by the uterus expanding and stretching to make space. If they are severe and/or accompanied by bleeding, it is important to go to the hospital to get them checked out
