Yes, usually by simply visiting the nearest Social Security office with a current picture ID issued by a state or government office, and your birth certificate. If you need a new card because you got married and thus you changed your name, then also bring the wedding certificate issued by the county you were married.

When I got married, I took my aforementioned documents and spent approximately 15 minutes waiting for a clerk, 10 minutes at the window, and had my new card with my new legal name in less than 2 weeks, at no charge.
