There is a leap year every four years. The extra day is designated as February 29th because February has the shortest number of days (28 in non-leap years). My grandfather was born on February 29th during a leap year. Theoretically, therefore, he only had a birthday every four years, so he celebrated his birthday on February 28th in non-leap years.

One reason is simply February is the shortest month of the twelve months on the Gregorian calendar (the calendar most commonly used by most of the world). Historically speaking, Leap Day (February 29th) was introduced by the Romans before they began using sequential numbering in their calendar. When they did begin to use sequential numbering in the late Middle Ages, Leap Day fell on February 29th.
