For PVT to SPC the gold rank is worn on the collar. For SGT and above the rank is worn on the shoulder boards.
Not anymore. The last uniform in which a commissioned officer wore rank on their collar was the Battle Dress Uniform/Desert Combat Uniform, which has since been replaced by the Army Combat Uniform, where the rank in worn on a flap on the front of the shirt.
It varies between uniforms. On the formal uniforms (Class A, Dress Blues), the rank insignia (three chevrons pointing upwards) is worn mid-sleeve on either side. If the trench coat is worn with this uniform, then metallic pin-on rank insignia is worn on the collar points. On the Class B uniform (dress uniform with either short sleeve button up shirt or long sleeve button up shirt), the rank insignia is sewn into black shoulder boards which are worn on the epaulets. On the phased out Battle Dress Uniform (BDUs) and Desert Combat Uniform (DCUs), the rank insignia was worn on the collar points, and could either be sewn on fabric insignia, or pin-on fabric insignia. On the Army Combat Uniform (which replaced the BDUs and DCUs), the rank insignia is fabric - black chevrons sewn onto an olive background. This is velcro backed, and is affixed to a velcro tab on the front of the ACU top. Enlisted personnel do not wear rank on the beret, but, on the patrol cap and kevlar helmet, it is worn on the front, either in sewn on fabric form, or pin on metallic form.
Wear it with any Class A or Dress Blue uniform. It is to be adorned around the neck underneath the collar of the white shirt, either below the bowtie or atop the necktie.
In the Boy Scouts of America, the rank badge is worn centered on the left pocket of the official uniform.

If the scout is a Cub Scout wearing a blue uniform shirt the Arrow of Light is centered on the left shirt pocket flap above the other rank badges (wolf , bear, etc). If the scout is a Cub Scout wearing the khaki (brown) shirt or a Boy Scout the Arrow of Light is worn centered under the left shirt pocket (below the pocket).
Ribbons are usually worn on US Army Class A uniforms. They are placed on the wearer's left breast, above the pocket, in "Order of Precedence". There are many lists/picture charts available everywhere.
2 bars on the shoulder epaulet is a Lieutenant; 3 bars is a Commander. It doesn't make any difference which uniform is worn.

On the current Army Combat Uniform (ACU), it is worn on a velcro tab on the front of the uniform. Officers and Warrant Officers wear their rank insignia on the beret, whereas enlisted personnel wear their regimental crest. Rank insignia is usually sewn onto helmet covers - however, this is often a moot point, as the placement of the rank insignia on the kevlar helmet often leaves it covered by the mounting plates for night vision goggles.
