Refraction of light is a phenomenon in which light rays passing from one medium to another change its direction. Refraction takes place at the interface of two medium of different optical density.

For a light of given wavelength and for a given pair of media, the ratio of sine of the angle of incidence to the sine of the angle of refraction is always a constant. And this constant is called the refractive index of the second medium with respect to the first medium.

Yes and no. Refraction is bending of the light beam and is CAUSED by the fact that light has to travel at different speeds in different mediums. When the light ray slows down or speeds up it bends at the interface of the two media of it is not striking the surface of that medium at right angles.
