There is no question that some alcohol abusers become paranoid (although it is far more common in those who abuse stimulants such as cocaine and amphetamines), but there is no definite indication whether it is caused by the alcohol or whether some people self-medicate existing conditions that include paranoiac tendencies. In any case, a debilitating degree of paranoia requires a good psychosocial workup and physical exam, and that cannot be adequately accomplished while a person is actively drinking -- nor for some time thereafter, in the case of alcoholics.
