I would presume a 4.0 in an honors program would be considered higher. If it's a 4.0 taking a course load of shop classes, PE and Yearbook it wouldn't be considered to be very impressive. A 4.0 while taking AP Math, AP English, AP history and AP Chemestry would be a little more impressive. If you compete with the BEST students it's harder to get a perfect grade.

No. If your high school offers weighted classes, which means an A is worth 5.0, the GPA may be higher than 4.0 if you make good enough grades.

As far as I know there is nothing higher than a 4.0. However, that is probably the reason some graduates are designated as graduating "with honors" or "with highest honors."
