The first is the play scene, which the King reveals himself as a murderer. The second is the present scene, in which Hamlet fails to kill Claudius. The third is the killing of Polonius in the next scene.
There are 4 Scenes in Act 3.
A3 s1 is often called the Nunnery Scene. Briefly, Rosencrantz and Guildenstern report that they made little progress talking to Hamlet. R & G tell Claudius and Gertrude that Players have arrived and a play will be performed. Claudius and Polonius hide behind an arras, and eavesdrop while Hamlet talks to Ophelia; Hamlet rejects Ophelia and tells her to go to a nunnery. Claudius says he'll send Hamlet to England.
A3 s2 is often called the Mousetrap Play scene. The Players do the play that Hamlet has arranged. Claudius rushes out in the middle of the play. Hamlet sees that as enough proof of Claudius's guilt so that he should kill Claudius.
A3 s3 is often called the Prayer Scene. Hamlet finds Claudius alone, but doesn't kill Claudius because Claudius is praying.
A3 s4 is often called the Closet Scene. It's complicated, and a short description doesn't do it justice, but briefly, Hamlet kills Polonius by accident, speaks at length to Gertrude, also, the Ghost appears again.
Hamlet wonders in a famous soliloquy whether it is better to live or die.
Hamlet tells Ophelia that he does not love her.
Hamlet goes to his mother's room to confront her.
Hamlet stabs Polonius through a tapestry.
Other than perhaps what he had been doing to Ophelia when his garters were around his ankles - probably the realization that Claudius had poisoned the cup his mother drank from, and the subsequent stabbing pain he put into the King's chest....
Rosencrantz and Guildenstern give their report to the king. Hamlet delivers a speech you may have heard of, starting with "To be . . .". Hamlet gets really mad at his girlfriend Ophelia and tells her to go to a nunnery (brothel). And that's only in Scene 1.
Hamlet not killing King Claudius, Gertrude believing the Claudius killed Hamlet's father.
The poisoned tip of Laertes' blade cuts Hamlet in their duel in Act V; thus Laertes is the direct cause of Hamlet's death.

The Shakespearean character Hamlet says the phrase "To be or not to be" as part of an immensely famous speech in Act 3 Scene 1 of the play Hamlet. Whether he "belongs" to the phrase is something else, and I'm not sure exactly what that means.
Act 3 typically begins after the climax or turning point of a play, movie, or narrative. It is the section where conflicts escalate and lead towards the resolution of the story.
In Act 1 Scene 3 of Hamlet, Ophelia agrees to reject Hamlet's amorous advances as her father instructed her to do.
