The last career to survive was Cato. They died in this order: Glimmer (tracker jackers) Marvel (Katniss' spear) Clove (Thresh and a rock) and finally Cato (muttations).

No, the actual bodies were sent back to their respective districts for funerals. The wolf muttations (called mutts) were most likely made by combining wolves with each tribute's dna, which could've been collected by the prep teams. There was most likely muttations made from Katniss and Peeta's dna as well, though they weren't unleashed because Katniss and Peeta were still alive and the point of using the tributes in the genetic experiments was to freak the other tributes out
Yes, Katniss did kill. In 'The Hunger Games', she killed Marvel (because he killed Rue) and she killed Cato out of pity (after seeing him getting clawed and bitten by the Wolf Muttations). In 'Catching Fire', Katniss killed Gloss (after Gloss killed Wiress),

Madge wasn't in the movie, so Katniss had to buy the pin in the movie. Several of the lines where not similar. When Katniss and Peeta where on the "roof" in the movie, Peeta didn't show Katniss the "protective" forcefield like he did in the book. Effie had pink hair in the book. The movie didn't show how Katniss familiarized Rue with Prim (I think they could have shown a flashback of Prim while Katniss was with Rue). The movie didn't show that the dog/wolf muttations had the eyes/hair color of the dead tributes. The "gifts" that Haymitch sent Katniss during the games didn't come with notes in the book. I think they should've had Katniss narrate the entire movie to fulfill many fans' hopes :-/
