Some causes of heart murmurs can include a congenital defect in a heart valve, a hole in the wall of the heart, endocarditis, and many other medical conditions. Some symptoms of a heart murmur are shortness of breath, fainting, and pain in the chest area.

valve not opening fully, rough edges on valves, constrictions of vessels or ventricles of the heart, increased blood flow (may well be normal ... or not)
You have four valves in your heart. Right atrio-ventricular valve, right pulmonary valve, left atrio-ventricular valve (Mitral valve.), aortic valve. All of they can become narrow because of 'steno-sis' or blood get in reverse direction called asincompetence. In addition, you may have inter atrial septal defect or inter ventricular septal defect. there may be patent duct-us arteriosus. All of them cause murmur and it is a harsh sound and once you hear it, it can be easily diagnosed. Of course, to diagnose which valve and what type of murmur is very difficult job.
