In my experience, lakes are natural bodies of water. But there is no legal reason known to me why a swimming pool should not have 'lake' in its name.

When swimming in a luxury pool, it is not as mandatory, however it is recommended in a more competitive pool. Less resistance the better.
It depends on the circumstance. A pool is better if you are farther away from the lake, and have enough space and money to install a pool at your house. It is also good if you like to swim alone, and not with other people. A lake is better if you are really close to a lake and do not mind swimming with other people.

Quite so. The water in a swimming pool is cooler than body temperature (unless the swimming pool is actually a hot tub) and therefore, a person in that pool will lose heat by conduction.
yes running the filter while swimming helps the filtration process. Also if the pool is saltwater then the system will replace chlorine being used while bathing.
Yes. Everything is disinfected in a chlorinated pool. I would be more hesitant before swimming in a lake or ocean, however.
