I think it just means to go to the toilet though there could be something else it refers to.
"To take a piss" means "to urinate", or "to go the toilet". On the other hand, "to take the piss" means "to ridicule" or "to tease in a derisive manner".
Thus, in a pub: to say "Excuse me, I have to take a piss" should lead to no altercation. But "Are you taking the piss" just might.
