Rivers always flow downhill. Given the vast expanse of the ocean, most of the time, if you keep going downhill, even on the slightest of slopes, you will eventually reach the ocean. There are a few exceptions, though, where rivers end at basins not connected to the ocean. Several rivers once fed into the landlocked Aral sea before they were diverted by human activity. The Okavango river simply spreads out, forming a delta in the Kalahari Desert, where most of the water evaporates.
A large body of water that flows through land might be called a river. Another name for a large body of water that flows through land would be a canal.
