You replace the radiator

Coolant goes in the radiator or you can also fill the radiator resevoir.
There is a water reservoir tank that goes to the radiator that you can add the water to.
In most cases the lower radiator hose feeds the water pump. The upper radiator hose feeds the radiator.
Engine could overheat, can bust your radiator hoses. If ran for long while overheated you could blow the engine.
When your radiator fails in a car, your car will overheat. Sometimes it will explode with steam. Usually you lose your radiator coolant. Often you will have to replace your radiator.
