Just like a disease or operation index, a physician index is a guide to identifying medical cases associated with a specific physician. Essentially, the data required in these indexes include the physician's name and code; the health record number; the diagnoses, operation(s), and disposition of patients treated; dates of the patient's admission and discharge; and the patient's gender and age. Often, depending on the application of the information, other demographic items could be included.
