It depends on whether the caster wants to become the fairy or if they want someone ELSE to become the fairy. There are many requirements to meet: the spell must be cast at midnight on the night of a full moon, the person must close their eyes and be facing in a way so that, if their eyes were open, they would see nothing that is man-made, and there must be music coming from a wooden wind-chime in the background.

The actual words of the spell are slightly more complicated. The effect lasts until the next night at midnight if you are making someone else the fairy, but if you are becoming the fairy, you will stay that way forever unless you cast the same spell on yourself with the letters read backwards. Included is all the spells needed.

For making someone else the fairy, say this: Tha eínai mia neráida méchri ta mesánychta áv̱rio

For making someone else human, say this:

Oirvá athcynásem at irhcém adiáren aim ianíe aht

For making yourself the fairy, say this:

Tha eínai éna paramýthi gia to mí̱kos tou gia pánta

For making yourself human again, say this:

Atnáp aig uot sokí̱m ot aig ihtýmarap ané ianíe aht

WARNING: this will not work unless said completely correctly, at the exact time. Also, Oberon and Titania (king and queen of fairies) must approve of this change from human-to-fairy or fairy-to-human.
