Yes the alternative to male circumcision is auto-circumcision, whereby training the foreskin to stay in a retracted position. Many males do this.

Note rarely some males are born without the appearance of having foreskin though it really is a very short foreskin. Some called this circumcised by an Angel:

Adult men in the western world can either be circumcised in the hospital or in specialised clinics and also some GP's practice circumcision. For adults local anesthetic is usually applied, although sometimes full anesthetic is applied, especially in hospitals.

Circumcision can be done "free hand" or with the aid of a circumcision device (usually a clamping device). Free hand circumcision is commonly used for adult circumcision.
The foreskin has an outer layer (this is the skin on the outside of the foreskin) and an inner layer. How the circumcision is performed, depends on which of the layers of the foreskin will be removed. The inner layer is believed to be very sensitive, reason for many men to decide to keep all or a large part of the inner foreskin.

In this style of circumcision, where most of the inner foreskin layer is preserved, a cut around the penis shaft is made in the skin between the base of the penis (this is near belly) and the glans. This separates the skin around the penis in a part still attached to the base of the penis and the other part still attached in the ridge just behind the glans. This part of skin is now pulled forward over the glans, leaving a part of the penis shaft behind the glans temporarily skinless The skin is then cut between where it is attached behind the glans and the place where the inner foreskin layer and outer foreskin layer join, next to the joint. This leaves the inner foreskin layer still attached to the penis, just behind the glans. The inner foreskin is now pulled backwards over the glans and its edge and the edge of the skin around the penis shaft are sutured together. Because the sutures are situated high on the penis shaft, this style of circumcision is called a high circumcision. The place of the sutures later becomes a more or less visible scar. Because the penis shaft is now covered partially with shaft skin and partially with the inner foreskin, a color difference may appear between the two types of skin. Some men dislike the visible scar or color difference on the penis shaft.

Another style is called a low circumcision. The circumcision scar lays in the ridge just behind the glans and is far less visible then with a high circumcision. Also there is no color difference. For this style a cut is made in the foreskin in the length of the penis, using scissors. Then the foreskin (inner and outer layer) is cut around just behind the glans and removed. Another cut is made around the shaft of penis, but much nearer to the glans as with a high circumcision. The now loose bit of shaft skin (sleeve) is removed. The shaft skin still attached to the base of the penis, is now stretched towards the glans, and sutured to the edge of the tiny part of inner foreskin just behind the glans. Now the sutures lay just behind the glans.

Together with the foreskin, also the frenulum can be removed. The frenulum is the band on the bottom side of the glans. Sometimes the frenulum is too tight, causing problems and this could be a reason to have it removed. Also cosmetic results can be a reason to remove the frenulum.

Circumcision can also be performed using circumcision devices. For adults the gomco clamp is often used. The foreskin is trapped inside this clamp, and then cut around a circular bell. The advantage is that the cut will be very symmetrical and is much easier for the surgeon.


It's usually easier in adults, because babies are tiny and adults are large.
