Doctors don't usually put you to sleep during a C-section unless the mother has heavy, uncontrolled breathing and can't handle the affects of the local anesthesia (which keeps you awake) or if there is no time to wait for the local anesthesia's effects to kick in

Use CPT 64920 if it is performed WITHOUT anesthesia, use CPT code 64921 if it is performed WITH anesthesia, and then the anesthesia cannot be billed separate, as payment will be included in the second CPT code.
You can have local anesthesia during childbirth, it will ease the pain, but not remove all of it. If you want to give birth without a c-section it's impossible to do it painless. C-section is painless during the birth, because you're under full anesthesia, but afterwards it still hurts, like every operation.
Yes, it has been done for millions of years. I wouldn't recommend a c section without anesthesia, though.
