Bisphosphonates are a class of drugs that prevent the loss of bone mass, used to treat osteoporosis and similar diseases. They are the most commonly prescribed drugs used to treat osteoporosis. They are called bisphosphonates because they have two phosphonate (PO3) groups and are similar in structure to pyrophosphate.

The bisphosphonates are powerful, they cause dramatic changes in the bone physiology, and they deserve respect. In women or men whose bone density T-score is lower than -2.5, or who already have a vertebral fracture, these medicines reduce the incidence of fractures and improve the quality of life. Evidence shows that they reduce the risk of osteoporotic fracture in those who have had previous fractures.
