People can have all sorts of problems, and chemical dependency is just one type of problem although it is usually a very serious problem. People can, for example, be unemployed and having trouble figuring out what to do with their lives, they can be unhappy with their Significant Other and in a state of conflict, they can be homeless, they can be schizophrenic, they can be afraid of Spiders (or pretty much anything else), they can have problems getting along with their parents, their neighbors, their employer, their fellow employees, and pretty much anyone else, etc. So there are lots of problems which might require counseling.
