"Girth" most often means the distance around some solid object, usually the smallest possible such distance if the object has a complex shape. However, the words "smallest possible" are often qualified by fitness for some particular purpose. For example, the "girth" of a horse would usually be measured around its back and stomach, where a saddle might be held, than around one of the legs, which would usually be a much smaller distance.
