In short:

Heroin:

First, some background: it is important to note that heroin is just a "trade-name" for the actual chemical;akin to"Oxycontin" being the trade name/marketing name for its active chemical oxycodone. Heroin just seemed to stick when it was first marketed. Diacetyl-morphine is the actual active psychoactive chemical in heroin. And as i'm sure you've guessed- if it were used in a medical setting w/ 99.9% purity (like other medically used pharmaceuticals)... it would be no more dangerous than morphine itself. The added "diacetyl" molecule just means in penetrates the more quickly, causing more of a rush, and thus, more abuse potential than say morphine.

Nearly all of the dangers of heroin is due to its unregulated status, being monopolized by the black market. So for one, its dangers are mostly due to undissolved "cut" (or additives, the point being to make more profit at the hands of distributors & dealers as to make their batch last longer).

So, aside from addiction potential (as is the case w/ ANY opiate or opioid). heroin would be no more dangerous than, say, morphine. Such dangers of black-market heroin use include degradation of veins, improper injection techniques (such as: missing the vein & shooting the diluted drug into the muscle tissue; mistakenly injecting into an artery rather than a vein; inadequate sterilization of the needle/injection site which can cause infection or blood poisoning; sharing needles possibly causing the spread of various spreadable disease like HIV, AIDS, forms of hepatitis; and/or injecting the drug into the vein(s) w/ air bubbles). ALO, such black-market heroin dangers include: criminal behavior associated w/ obtaining the drug due to its inelastic black-market prices that are highly inflated; stroke; heart attack; & gangrene/limb amputation (the latter tree of which are due to "cut" added to heroin). Another problem that is directly due to cut in heroin when injected is liver and/or kidney damage!

Otherwise, medically pure opiates/opioids are quite safe to the body (although withdrawal is HELL). There are SOME exceptions however, such as propoxyphene (or Darvocet/Darvon), but this is due to its local anesthetic effect (like cocaine),all of which, local anesthetics that is, are known to be toxic to the heart. But that aside, pure heroin is not toxic to the brain, heart, kidney, liver, colon, nor the intestinal system.

Cocaine:

Conversely, cocaine is extremely toxic. First, it damages the heart & cardiovascular system. This is because for one it is a stimulant, & long term use of any dopamine-acting stimulant will cause such damage. Additionally, all local anesthetics are damaging to the heart & can cause heart enlargement w/ persistent use; and cocaine is definitely a numbing agent, or local anesthetic, and as such this adds to its dangerous effects to the heart.

Its impacts on the heart & cardiovascular system can also cause death after a single use--not bein a result of overdose either. If an individual's heart is sensitive, the actions described above can cause death.

Also, injection of cocaine has the same problems as IV'd black-market heroin. And, due to its direct impact on dopamine (like any other illicit stimulant), it is neurotoxic to such receptors & can cause long term changes in the brain as result of extended abuse, even after years of abstinence. Such issues can cause Parkinson's like issues, psychosis, dementia, schizophrenia (called "amphetamine"...or "cocaine psychosis. However this tendency is usually only during use of the drug). Extended insufflation (or "snorting") of cocaine can additionally cause damage to the sinuses which can result in face collapse around the nose in extreme cases, smoking cocaine can cause extreme lung issues (including Asthma at the least) even more so than tobacco use if cocaine is smoked for long periods of time.

But the major detrimental effects cocaine use can cause is in the brain & heart (as explained above). A ere one-time use in unknowingly heart-risk individuals can cause heart attack; and also stroke. Thse problems are present despite purity. The only problems that are a result of "cut" to cocaine (or additives) is when it is injected.

Now I can't specifically answer your question as to "why" cocaine is toxic, beyond what i've already stated. In short, why it is so dangerous is because of its pharmacology... that is the best I can say beyond waht i've already said.

To Conclude....

As a recovering opiate/opioid addict (while also indulging on cocaine binges once in a while), i'd highly recommend not using either of these drugs. That is, if you haven't already (and if you have already used them, stop while you're ahead... if you're not dependent upon them). The few moments of joy fair outweigh to potential risk of addiction & dependency, which I can guarantee you will ruin your (and your family's.,.) lives.

Toxicity on Kidneys. After doing cocaine for only a couple of years, a kidney tumor that had been stable at 4cm not only doubled to 8 cm but caused complete urinary stoppage with huge blood clots in between. The heroin would not cause blood but just one toot now causes the bloodiest of urinations preceded by hours of pain and burning.
