Sodium bicarbonate is used as an IV solution in some emergencies such as cardiac arrest, circulatory insufficiency due to shock or severe dehydration, and in severe primary lactic acidosis or severe diabetic acidosis.

It may also be used in some cases of drug overdoses, including barbiturates (where dissociation of the barbiturate-protein complex is desired), in cases of poisoning by salicylates (like aspirin) or methyl alcohol poisoning. It also may be used in cases of severe diarrhea which is often accompanied by a significant loss of bicarbonate although this might not really qualify as an emergency.

In some cases it also works to administer the sodium bicarbonate orally to help with treatment of poisoning - for example - to inhibit absorption of aspirin in cases of aspirin overdose by raising the pH of the stomach and gut contents to a more neutral level (aspirin needs an acidic environment for absorption).
